Who We Are
Concept Care is a Sydney-based, NDIS registered disability service provider, delivering high-quality, person-centred services in Complex Care, Mental Health, and Behavioural Support. We are committed to helping individuals live safer, more independent, and fulfilling lives—regardless of their level of need.
Our team includes trained Support Workers, Clinicians, and Care Managers who work under a 24/7 support model. We focus on compassion, professionalism, and respect in everything we do.
What We Do
We offer a broad range of flexible and individualised services, including:
- Complex Care – Clinical support for palliative care, hospital discharge, and management of chronic or degenerative conditions.
- Mental Health & Psychosocial Support – Recovery-focused services that support emotional wellbeing and long-term stability.
- Complex Behavioural Support – Assistance for individuals with behaviours of concern, including Behaviour Support Plans and strengths-based strategies.
- NDIS Support Coordination & Case Management – Helping participants navigate their NDIS plans, manage providers and budgets, and build independence.
- Assistance with Daily Living – Personal care, mobility support, hygiene, domestic tasks, and medication prompts tailored to individual routines.
- Community Access & Transport – Support for community engagement, attending appointments, and travelling safely and confidently.
- In-Home Support & Respite Care – Consistent care in the home, including short-term or emergency respite for families and carers.
- Private & Holiday Support – Flexible, non-NDIS-funded support during holidays, transitions, or special occasions.
- Day Programs & Recreational Activities – Inclusive activities that support social connection, creativity, and personal growth.
Our Journey in 2025
This year has been a significant chapter in our growth. We’ve welcomed new clients across Greater Sydney and regional NSW, strengthened partnerships in allied health, expanded our clinical teams, and continued investing in staff training, technology, and care quality.
In 2024, Concept Care was featured on the Australian Financial Review’s Fast Starters List, recognising our rapid growth and service excellence. In 2025, being named a finalist in the Camden Local Business Awards highlights something even more meaningful—the relationships we’ve built and the lives we continue to support.
